Sure it  will take a few minutes to fill it out but it should save a lot of  time with emails and all my questions.  You can fill out as little or as much information as you like.  Click  Submit at the bottom of the page and the order form will be delivered to me as email. For team orders and pricing You should contact me by email. You can open a new browser window and come back to this page.   From your order form I then post examples for approval.  When we settle on the design then  I usually mail the same day. They cost 20.00 each and the US postage is included.  ( for  first class mail )  I include instructions and a self return envelope so  you can send me your payment.  I accept just about anything.   Paypal  is also accepted .

About Your Handlebars This is important,  So I know where to put the attachments. Other wise I'll be asking. Select Type Pro size, Expert, Junior, Mini, or Cruiser Pro Size Expert Size Junior Size Mini Size Cruiser Type Do your handlebars have a crossbar?          NO YES My Bars measure 1- top length of crossbar  2- the height (from the crossbar down to the bottom) 3- bottom width. by these measurements I connect the lines to determine where to put the holes/zip ties. Measurements can go here:       Or if your handlebars have a brand name and model etc. But I do prefer the measurements. I suggest Velcro Straps  They are attached with aluminum rivets. Choose one.  Yes, I want a flag for my  background No Flag 1 2 3 4   #3 Background Colors This is for the background behind the numbers. Usually it's white but some riders have gray or black. Refer to ABA or NBA rules. (Solid Color)  if you wanted a solid red, blue etc. Type in  the color.  Leave it blank for white behind the numbers

 

Flames Flames On the Bottom and sides? (red and yellow)          No Flames Flames on the bottom  only Flames on the sides  only Flames on the bottom and sides   If other than red and yellow flames then type in the colors Flames on the inside of the numbers?  (red and yellow)           No flames inside of the numbers Yes flames inside of the numbers If other than red and yellow flames inside of the numbers  then type in the color.      No Yes Checkered flag in the background?  If you're getting blended colors in the back ground (not a flag) then I suggest the checkered flag

 

Did you want a number on your plate? Some people don't want a number because the plates are lasting  more than 1 season.  Also if it is near the end of the season you may want to consider not having a number.
